I/We will attend and/or give my/our permission for the student mentioned above to attend Cremona International Music Academy (hereinafter “Academy”) conducted by the International Music Academies (hereinafter “IMA”) from July 11 – July 28, 2023 in Cremona, Italy.

Academy programs may include public performances and permission is hereby given for the students to take part in such performances without compensation. By registering for studies at the Academy I/We grant the Academy and IMA, and those acting on their behalf, the authorization to a) record all students’ work, including public performances, masterclasses, and workshops, record students’ participation and appearances on audio and video media, photograph, film or other mediums; b) Use students’ names, voices, biographical materials in connections with these recordings; c) Exhibit and/or distribute such recordings in whole and/or in part, without restrictions and limitations, for any educational or promotional purpose that the Academy and IMA, and those acting on their behalf deem appropriate. I/We agree that all rights in and to any and all recordings made of students’ performances, including all video media, audio media, film, and photographs shall be owned exclusively by the Academy and IMA.

I/We understand and accept the measures that the Academy and IMA have taken to provide a program of the highest level of quality, and I/We accept that my child will be expected to behave accordingly. For all students younger than 18 years old, the curfew is in effect from 11.00 pm (11.30 pm on the night of a concert) and they MUST be in their rooms at this time. No visitors are allowed in rooms after 11.00 pm (11.30 pm on the night of a concert). Also, no overnight guests are allowed. Room‐check is conducted nightly, and violations are reported immediately to the directors. Should any damages caused by my child be made to equipment and/or rooms that will require repair and/or an excess cleanup I/ We are willing to incur the expense of that repair.

The Academy and IMA reserve the right to terminate the participation of any student for serious violations of the Academy and IMA Rules of Conduct policy. Students are not permitted to possess and/or consume illegal drugs and alcoholic beverages of any type. If my child participates in behavior deemed unacceptable by the Academy and IMA, or its representatives and staff, the Academy and IMA reserves the right to expel him/her and be sent home immediately at the family expense and without refunding any paid fees.

The Academy will contact Parent/Guardian in case of serious sickness or accident. In the event that I/We cannot be reached in an emergency, I/We hereby give permission to the physician selected by the Academy and IMA staff to hospitalize, secure proper treatment for, and to order injection, anesthesia or surgery for my child named above. I/We also hereby give the Academy and IMA permission to administer medication deemed necessary by the physician selected. I/We understand that the Academy and IMA assumes no liability for injury and accidents that may happen to my child or his/her belongings while traveling to and from the USA and other countries or during their stay in Cremona, and also while traveling to and from performing venues and to and from excursion destinations.

I/We understand and agree that as a condition of the participants uses of the facilities assigned to the Academy, he/she assumes all risk of personal injury, death, or property loss resulting from any cause whatsoever including but not limited to the inherent risks of the program, breach of contract or breach of statutory, a duty of care or breach of the Occupier’s Liability Act on the part of Academy and/or IMA and/or Cremona representatives, its employees, and agents. The participant agrees that its employees and agents shall not be liable for any such personal injury, death, or property loss that occurs outside the program parameters and releases the Academy and IMA, and Cremona representatives, its employees, and agents of all claims with respect thereto.

COVID-19 GOVERNMENT REGULATIONS

Cremona Academy strictly adheres to all USA, Italian, and EU governments’ restrictions, protocols, and recommendations on health in respect to Covid -19. The Academy will closely monitor all developments and will update the website regularly. The information will be provided about any possible changes should they be made. Please make sure to check travel and transit requirements, and entry procedures with your airline/ travel agent, and/or Consular representative.

I/We accept to follow and adhere to EU/EEA COVID-19 Health and Safety protocols, including but not limited to being fully vaccinated, providing proof of a negative Covid test prior to travel, carrying all appropriate paperwork and forms as per EU/EEA travel regulations and entry requirements regarding international travel to, from, and within the European Union.
